Northwest Integrated Health is seeking a Spoke Care Navigator (SCN) who is empathetic, charismatic, a team-player and passionate about helping a complex and vulnerable population working for the Washington State Targeted Response (WA-STR) Hub and Spoke Project. The SCN can be a chemical dependency professional (CDP), Social Worker (SW), Registered Nurse (RN), Licensed Practical Nurse (LPN), Medical Assistant (MA), Community Health Worker (CHW), Certified Recovery Coach or Certified Peer Counselor.
ABOUT THE PROJECT:
The Division of Behavioral Health and Recovery (DBHR) received funds from the Substance Abuse Mental Health Services Administration (SAMHSA) Targeted State Response Grant to develop six Hub and Spoke (H&S) projects in six areas of the state. Funding for this project is part of a commitment from the 21st Century Cures Act to address the Opioid Epidemic.
The H&S model creates a coordinated, systemic response to the complex issues of opioid addiction among the Medicaid and low-income populations, focusing specifically on Medication Assisted Treatment (MAT) for individuals with opioid dependence. The integrated care model connects a network of community providers around a central hub that offers a MAT component to all patients seeking services for opioid use disorders. Northwest Integrated Health is the Hub located in Pierce County.
The SCN will be employed by Northwest Integrated Health, and will be assigned to one or more of the chosen Spokes: CHI/Franciscan Emergency Departments, Greater Lakes Mental Health, Olalla Recovery Center, Pierce County Alliance/Pierce County Drug Court, Thurston County Drug Court, Tacoma Needle Exchange, Puyallup Tribal Health, Nisqually Tribal Health, and the Pierce County Fire Districts.
The SCN will be the Spoke’s point of contact for staff who have patients that are interested in MAT services and will facilitate referrals to the Hub for MAT, withdrawal management services, chemical dependency counseling, and/or mental health counseling. Will also provide case management for primary care, employment services, transportation, housing and food resources. The SCN will provide MAT education and training for staff at the Spoke. When necessary, represent NWIH in professional meetings within the community.
2-3 years of experience in a healthcare setting, preferable chemical dependency
Preferred knowledge of EPIC and ability to learn and utilize health management information software
Ability to operate a computer, fax machine, and multi-line phone
Previous experience with Microsoft applications - Outlook, Excel, Word, etc.
Understanding of insurance eligibility and verification processes and knowledge of billing guidelines
Effective time management and organizational skills
Position requires extensive knowledge on the use of Motivational Interviewing
All personnel are required to maintain credentials
Must have a valid WA State driver’s license, car insurance, and own a vehicle
Pass a WA State Background check
